"family" is an in-character term.
"user" is an OOC term.
Which means your family relations can be considered well-known. They
would, in a medieval world where blood was thicker than water. You can
make an exception for adventurers who, after all, are bastards. They
might not be recognized, in that case the relationship is OOC (you
know he belongs to the family, but the family denies and while your
character might have heard rumours, DNA tests didn't exist so no one
really knows for sure - ah heck, just make it a good roleplay!)
