This shows my issue quite clearly.  Suppose I just want to form a guild. 
  I have no interest in being a region lord - I don't want to deal with 
oaths and knights and taxes and whatever else comes with being a lord. 
I just want to run a guild and promote it around the island.

Someone else wants to be a lord.  They want to deal with oaths and 
knights and taxes and whatever else comes with being a lord.  And they 
have no interest in making guilds.

But either they are prevented from being an enthusiastic, 
good-for-the-game region lord, because I'm hogging the region lord spot 
just in order to make a guild, or I'm prevented from making a guild, 
because they're hogging the only guild-making spot, just in order to be 
a region lord.

I simply don't see the link that lords should be the ones that form 
guilds.  The people with time to put into guilds are quite possibly the 
people who have nothing else to do - ie those that are not already 
region lords.
