they do. sort of. in republics and others that hold elections, religions
can (assuming they have enough noble followers) put up their own
candidate. and if the realm is mostly run by council/senate of region
lords, they could try filling the council with their own followers.
of course, just because someone follows a religion doesn't mean they do
whatever it is that their elders say.
The other thing would be... realm is still the focus, rather than
region/duchy (or religion for that matter). So loyalty to realm is still
rather pervasive. Chances are people will just quit religion rather than
change their realm.
