Yes, in RP it is all good.  But this is also a game which people want to 
play to enjoy.  To make the RP more sensible, surely if a King dies, he 
should be replaced by his next of kin (ie another character of the same 
player).  This would make more RP sense, but I hope you can see it isn't 
great for the rest of the players of the game for one player to be ruler 
of a realm forever.  Don't let RP and realism force you to have a game 
that looks lovely but is crap to play.

I don't see that it would restrict the RP very much.  Just means you 
have to think of a reason through RP why you can't let the Judge be a 
Duke.  And for players who gain a position, it will enhance their RP, as 
they'll have more to RP about.

So the solutions are to limit the number of positions per player, or 
increase the number of positions.  Or make it so that people with no 
position have just as much to do as people with a position.

It's not more boring for the people who get to hold a new position.  And 
saying that it must be done by the players, with no rules from game 
administration, is like saying there's no need for governments to set 
laws, or police enforce them, but society should just be nice to each 
other without assistance.

It would be more realistic only to fight in the daytime.  It would be 
more realistic to have messages take days to arrive.  It would be more 
realistic if there were some days that the weather was so bad that 
no-one would dream of fighting.  It would be more realistic if all 
nobles could be killed in battle.  It would be more realistic if my 
characters sometimes just got the plague and died a horrible death.  But 
each of these would make the game more crappy and less enjoyable.  IMO, 
realism is a "nice-to-have" feature.  Gameplay is a must.
