This is because of the way the links are created. They are referenced by ID
numbers, but the IDs are not unique to that island. f.ex: All islands have a
region "20". If the last character you accessed was on Atamara, and someone
passes you a region link, it will show you the region with that ID number of
Atamara.

Probably population. Kingdom of Alluran owns the largest city on the island, and
has over 7 times the population of Thalmarkin.
There really is no other way to base the "size" the realm. Population
determines, in a rough way, the income of the realm and the number of nobles
that realm can support. Going by a simple region count would be even more
misleading, as a realm can have many small and poor regions.
