I think the most important key that is limiting religions breaking
away from their founding realms is that priests can be arbitrarily
arrested like peasants or adventurers. As long as a priest stays
inside his realm - and very often the realm where the religion is
founded - it is kinda certain the priest can enjoy the hospitalities
from his own fellow nobles and won't be touched (unless the priest
proves to be a sucker himself). However as soon as the priest steps
across the borders, he can be trashed anytime, not even before he can
convert anything to his belief to protect him from the apprehension.
I would say this part should be supplemented with some more generous
conditions. For instance perhaps make the "arrest priest" option
available only when the priest _has_ conducted something "bad", like
prosecution or badmouthing etc., so if the priest is just out there to
preach and construct shrines to spread the religion, no one can stop
him. No need to code some extra penalties that kinda artificial and
specific towards the phenomenon.
