They can become actual nobles. They need a certain amount of honour and
prestige for that. Number 2 is completely valid, but I don't believe
you can get a recommendation from another realm.
I understand that they can become actual nobles but it would seem to me that something as simple as acquiring H/P doesn't mean anything to the noble characters. They have no knowledge of your exploits as a worthless Sack o'Crap that goes around killing undead and monster packs. The local region lord doesn't know that adventurer Bob just spent 16 hours chasing a pack of hungry monsters around his region, he just knows that there are no monsters around. And he doesn't really care who did it because he's off fighting a war somewhere or he's in bed with whatever young lass he could get there. The only reason a noble cares about a adventurer is when they pester them to buy something the adventurer calls a "special item" otherwise they are well beneath their status to mingle with or talk to. And if the adventurer can do enough of a 'sales job' to the noble, the noble might be feeling very generous and bestow a recommendation upon said adventurer...but these should be very
few and far between.
Most interaction I've seen from adventurers to nobles go something like this:
Bob: hey, region lord. you wanna buy an item from me for <xx> gold? And give me a recommendation for selling it to you?
RL: I should have you thrown in prison for talking to me that way.
Bob: well if you don't want to buy it, just say so. OR Ok, how about for a bit less gold? And a recommendation would be great.
And since my RL character can't immediately remove the head of said Sack o'Crap and keep the special item, the SoC merrily goes on his way to swindle some other RL out of a recommendation.
