some brainstorming

Try..

More penalties for moving capital.
More penalties for starvation.
Making duchy secession easier.
Improving on the estate idea, make people really like their land.
Add attrition so armies can't mass easily without losing men to sickness,
weather etc.
Let duchies have relations?  Maybe duchies should be able to fight, inter or
intra realm, without it escalating into war?
Federations should have someone elected to 'lord' over all nations in it.
maybe an emporer.  He could have little power, but annoying power and, this
could spread animosity since people of realm b may not actually want to take
orders from realm a, and more federated politics increases chance of civil
war.
