What occurs to me:

  First of all, a country shouldn't even need to be in a realm to
appoint a lord for it. They can do that anywhere and at any time.
Extremely ambitious countries could even pretend that they will
eventually control an entire continent. It comes with no benefits but
being able to put a title next to your name, though, unless you
actually establish control of a region.

  You'll end up with, especially in contested realms, parallel power
structures. Next, make a knight's default tie to a region much more
clear. Give each one an estate with the first level of investment
already done in their starting region right out of the gate. Now while
the lord has control over which knights can build an estate in a
region, knights with estates in a region should have direct control
over the recognized lord of that region- set them to vote for which
among them is the lord of the region. (This will discourage lords from
taking on new knights that might put them out of control, obviously.)

  When an invading army moves in, they can, by force, begin to install
their own knights in the current estates- that's what the takeover
process should be, every round, depending on the force available,
they're able to clean out a certain value worth of estates (Bigger
ones are harder to take over, or maybe have to be reduced down a few
levels before they can be taken) and install their own knights in
them.

  Knights who don't want to lose their estates either need to come
actively defend them, or swear to the new lord of the region.

  While the war is on, income in the real is not only reduced, but
split based on number of estates controlled.

this a a great idea I only hope that it is not too hard to code.  the possibilities of this could turn a war  direction over several times and would give the art of politics a very large part in all wars.

If you were able to convince knights or even a lord to turn over to your realm and there by making the TO switch over faster the blow of the region lost and men would be tremendous in most cases.

epically if the decisions of the lord to switch over could make the realms claim worthless!
