I don't know about "In most realms".  A large number of realms have only one
Duchy.  Such is the case with 2 of my realms.  The one my adventurer is in
has 2 cities, and it's a fairly even spread between the two.  The largest
realm I'm a part of has 3 duchies, and the smallest one has 3 regions and
about 8-10 nobles as a part of it.

So I'm not sure if it was an exaggeration or just being misinformed.
Regardless, I like the idea.  Expanding a little further, Army leaders could
potentially do the same thing.
