They arn't taking an extra step nessisarily...
Reply to 1: 1 click
Send to all of realm: 2 clicks
Reply to a small group within the realm (a convo of 4 or 5, I've seen
it often) 3+ X clciks, with X eqiling the number of people you're
sending to... not to mention scrolling through a list to find their
names.

So sure, a 1 on 1 convo is easier to be private, but any more then
that and its MUCH easier for the group to simply talk to the wholre
realm.
