The perhaps "inheritance" should be an option in appropriate government styles - Monarchy, Tyranny, etc.

I would say that dukes/counts/etc are inherited titles, it's just that
everyone dies without an heir and the title goes extinct, because
there are no heirs in BM. Your right to a title is the same regardless
of whether it was created for you or you inherited it.
