I think it is a great idea to have question nobility turn people into
adventurers. It brings the mechanic in line with the description.

Sounds very reasonable. My character didn't have a duel to the death option
though (only to surrender). Bug?

This also works well.
I would add:
3) there is a code that determines whether or not it works (like the code
that determines success for TOs). Various factors affect it, but in the end
the result is hard to guess. Factors would include:
-honour/prestige of the target (the higher, the more difficult)
-positions held (again, the higher, the more difficult)
-other family members in the realm/island (easier if the character is alone)
-family fame and wealth (the higher, the more difficult)
-time in realm of target (the longer, the more difficult)
-ruler's honour/prestige (the higher, the easier)
-ruler's length of reign (the longer, the easier)
etc.
Basically the better known/established the victim, the harder this should
be, but the consequences should be harsher (ie bumped down to advy, honour
and prestige wrecked).
