That and a lot of realms tend to just recycle duchies among the 
same group of characters. Indeed, a lot of the lord/council 
positions tend to be handed out this way. Once characters start 
running with the movers and shakers in any realm, they tend to 
stay there.

In a way, this is understandable. It's natural for these people to 
stay in power, the same way they got there in the first place: 
Being active, giving good advice, and taking whatever they can 
get.

Unless these same people willingly decide to step back and keep 
out of the inner circle, it's almost impossible to get them out.
