99% of cases? Honestly, I think I only had ever heard of it once when it 
was mentioned on this list (and then you decided to change it). 
Obviously I don't see anything like all of BM, but certainly more than 
1% of realms - unless your definition of "case" is "one time in Minas 
Ithil". And in any case, how is less turnover better than placeholder 
rulers? At least with the latter there's a chance for things to go pear 
shaped (in a week, I'm sure even a thronewarmer king who put his mind to 
it could entirely change sides in a war if he wanted).
