I also have a problem with this.  If an 8 year old kid walks up to you
and says you're the stupidest person he's ever seen, will you be
absolutely devastated?  Likely not.  If one of your peers, or your
boss says something similar, will it get to you?  Almost certainly.
An insult is only an insult when someone whose opinion you care about
says it.  Here in the US, and likely worldwide, tabloids publish silly
things about celebrities constantly, absolutely outrageous things.
How offended do you think those celebrities are by that?  Likely not
very, because the tabloids credibility, relative their own, is not
enough to trigger any mass-change in opinion.  Sure, some people will
believe them, but its beneath their notice.

It is a similar situation here.  If another Duke, or the Ruler, or
someone who has that Duke's respect insulted him, then his honour is
wounded.  But if an unaligned noble, essentially someone highborn who
has neither proved nor earned anything for himself, starts up the
insult machine...The Duke will just blow it off, and does not need to
be concerned about refusing a duel.
