I like most of them, though I would really like for the ruler title at 
least to be customised.

Also, the whole grand duke thing gave me an idea - maybe make it a title 
for the duke with the most regions in his duchy?

Could only be for flavour, or if you want to make it more desirable, you 
could split the 2 duke fame points into one for duke and one for grand duke.
