I don't know the majority of the realms I'm in have no rules for or 
against it.  One of my characters routinely beats adventurers up for 
talking to her without her addressing them first.  She recently had one 
arrested and hung for plotting a rebellion, with very little proof.  I 
wouldn't say it's a majority.
