[Discuss] Adventurer to knight?

Okay, so how exactly does this work?  I've wandered around the wiki 
quite a bit, and while it's possible I've missed it, I don't know where 
else it might be.

Other than the three recommendations, what requirements are there?  I 
heard somewhere (probably on IRC) that it's 5h/5p.  Is this true?  And 
if not, can anyone confirm what the requirements are?

And then, button clicking-wise, what needs to be done?  My adventurer 
has the three recommendations, 7h/5p, but can't find anyway to offer an 
oath or anything.  I've talked to a couple of lords, and they can't 
either.  Is it necessary to be in the same region as the lord?  Or is it 
just that the adventurer hasn't met all the requirements yet?

I'll add this stuff to the wiki if someone can provide the info.
