There were no forged _titles_ in the middle ages. None.

The only possible thing you could've done that at all resembles one is to 
forge a pedigree, such that, on the death of the existing lord, you would be 
next in line rather than the other guy who was next in line -- but without 
the political support of the overlord (Duke or King) even that wouldn't 
matter. There was nothing you could unilaterally do to guarantee yourself a 
title.

If it was discovered after the fact that you'd forged your pedigree, maybe 
the overlord would keep it quiet and maybe he wouldn't. It would reflect far 
worse on the forger than on the overlord if it was exposed.
