No, they would assume _all_ titles are legit.
Why? Because once your start doubting any titles, you are shaking the
very foundations you are living on. Who's to say that _your_ title isn't
faked? Why are you crying so loud? Maybe to cover up a skeleton in
_your_ closet?
I'm pretty sure in the real middle ages, a forged duke or baron title
would have been dealt with as quietly as possible. I know for a fact
that in the real world many people were actually promoted to positions
of power because they had successfully faked them, and revealing them as
frauds would have been more damaging to the system than keeping them.
