That's not true.  Most realms (that I know of) have no such formal  
procedure for granting titles--it's just done as the Ruler, or  
perhaps the Council, sees fit.

That's not to say, as you point out, that they won't claim that  
whoever it was violated their procedures, too...but nowhere else  
*I've* seen have it as codified as Riombara does.
