I'm of two minds about that.

1) If the item is never created again, then it is indeed Unique. You can be the 
only one to ever own the Golden Widget of Wherever.

2) If items are never found again, then they will never have more than a 
simple, isolated story associated with them. There will be no chance for more 
players to get involved with creating a history for that item. For example, 
Dimian, one of my characters, owned the Scroll of Good and Evil while Arch 
Priest, and still had it when elected as King. So now this item was "in the 
treasure hordes of kings". Other players can build on that and add more 
chapters to the history of the Scroll.

So I think that if an item is only ever created once, then we are missing an 
opportunity for more story-telling.
