My own reasoning against it would be that not everyone deserves
personalized titles.  And especially not ones that you come up with on
your own.

Now, if there were an option in place for, say the Ruler to hand out 3
personalized titles, Dukes to give 2, and regional lords 1 apiece-
then we might be onto something.  Can never give yourself or your own
family a title.  Makes them a fun addition without it being too
prevalent or abused.

The other concern is those who would have ABC, Minister of Defense,
Count of XYZ, Marshal of the Army of the Largest Named Battalion in
all the World, He Who Spits on Viking Men, Women, Children, and
Gravesites.

You get the idea.  Titles are already obscenely long for certain
characters.  Adding personalized titles in addition to all these would
only make things worse.  Even giving a character limit to the
personalized titles only does so much in the big scheme of bloated
titles.
