You can buy a title from outside the realm, it just costs a lot more and 
switched the entire region to your realm.

Maybe it wasn't especially premeditated? After all, given the time it takes 
to emigrate he would have to have known 3 turns in advance that that 
particular city was going to be available to buy. Again, time which could 
easily have been used to avoid the problem buy appointiing a duke.
