That's not how it happened (though I have only seen this happen once and
haven't been around for all that long to know how it's gone historically).
The Duchess of Colasan seceeded from Lasanar, taking Colasan (city), Isular
(rural), and Ossaet (neighboring city, part of a different Duchy) with her.
Our (loyal) Duke of Ossaet was automatically removed, and Lasanar had no
claims on any of the three regions. The noble appointed by the Duchess of
Colasan (now Queen) as Duke of Ossaet then seceeded from the new realm,
creating his own realm just in Ossaet.
After conquering all three regions, the seceeded realms (none of which exist
anymore) still have claims, and we cannot appoint lords to any of them. If I
recall from negotiations with the seceeded Duke of Ossaet, it wasn't
possible to negotiate a surrender with a realm that had only one region (as
it's apparently impossible to hand over the last region of a realm) so he
had no incentive to do anything other than sit there and let us come for
him.
It's been a little more than a month for Colasan and Isular, and about three
weeks for Ossaet since their respective secessionist realms have been
destroyed -- each of which existed for 2-3 weeks.
