Most realms I've been in have reasonably even duchies. At least by
regions - the capital duchy usually has more knights than any other,
since people like to swear allegience to the duke of the capital.

Tom's plan for a new hierarchy system (explained on the wiki page
"Heirarchy") should help that, as lords will be expected to provide
for their vassals, so it should be evened out both as the knight/lord
level and the lord/duke level.

The real problem is, as was said, that the strategy of throwing
everything you have at one enemy region, probably with all your allies
as well, is generally the best way foward. (Yes, there are exceptions,
but not many) Battles with large numbers of people need to be made
less efficient, not just in terms of winning the battle, though
(whoever has more men ought to win in most cases, if there isn't
something else swinging it). Prehaps there should be a greater chance
of getting killed rather than just wounded if the battle is larger
(too confusing for the strecher bearers to get to people in time). Or
a high chance of soldiers deserting in large battles ("There's so many
of us, I'm not making any difference, I'm off!").
